If you have signed up for any off-campus events, you must complete the Release Form and return an original, signed copy to the JWHC Office in Goodman 203 at least one week before the event. Anyone under 18 also needs parental consent. If you fail to complete this form by the deadline, we will give your spot to another student who has turned in his or her form.
